If I was Judge DeSalvo I think I would have ruled exactly how he did in the book. After hearing all of the truth about Kate being the one to really want this I think he did what is best for both of them. Both Anna and Kate wanted to not be connected and didn't want to donate and receive the kidney. But at the same time neither wanted to lose each other. It is a very hard decision and very hard on both sides of the ruling. Sara is more of a mother then she will ever be a lawyer she gave up on law the moment she had her first child. Being a mother is the strongest person she will ever be. She has the most important job as the role and its to protect and take care of her children. If she is forced to not be as protective in order to take care of another can you blame her. She is protecting Kate from death and taking care of her. But this doesn't mean she wouldn't have done the same for Anna if it was Anna sick instead of Kate. But as for Campbell he is different, he is just doing his job. He doesn't look at Kate's side of things he is fighting for Anna and only her. He understands what can happen if he wins and he understands what happens when he looses. But he was payed to fight for Anna no one else.
